Customer Reviews and Testimonials

Everyone understands the importance of Customer Reviews or Testimonials. Using Responsive Directory Theme, can anyone share success they have had with any specific Review or Testimonial plugins?

Requirements:
1) Ability to add to the directory listing
2) ability to add to any page on the theme
3) ability to integrate both comments and a star rating capability
4) custom fields would be nice
5) some protection from spam is needed
6) ability to limit per ip address, so one person cannot bombard the reviews
7) ability for admin to approve (or not) any comments or reviews before they are posted
8) ability to integrate nicely with the responsive directory theme

Any suggestions you have on something that works, will be much appreciated.
