Blank Details Payment Form plugin cannot be downloaded – Help!

I am trying to add the Blank Details Payment Form plugin (version 1.1) to my responsive Shopping Cart theme but after downloading the plugin, I cannot see it in my plugins list to activate it.

I also downloaded the Blank Payment Form plugin (version 1.0) and this appears in my plugin list. After activating it, nothing happens. I cannot see this as an option in the Payment Method of the Checkout page.

I wish to show customers my Bank Account number so they can transfer the payment manual to me. How and where do I add this information? On the Installed Payment Gateways page, I see the Blank Payment Form but it only has 2 fields – Enable/Disable and Email address.

Is there some step I am missing out here? Please advise.
