Adding Check box to check out page

    Hi,
    I would like to add a required field to the checkout page. It would be a check box to have the shopper confirm their age. Is this possible. I did add the terms and conditions plugin but that doesn’t add a checkbox to the checkout page.
    Can anyone help?
