City dropdown in advance search is not populating

Wordpress 4.1

Before I purchased this theme, I was using the Premium Directory WordPress Theme and successfully followed the tutorial here, https://docs.premiumpress.com/doc/setting-up-countrystatecity-searches/ to setup the City/State advance search. It was working as expected.

But when I use the same setup and server with Responsive Business Directory theme, the city is not populating.

The site is not live yet, so here’s a screenshot to help illustrate the problem:
