Add another Contact Form and a plugin for Feedback

Hi

Is there a way to add another customised Contact Form to the listing page, with customised title and fields. It would be another tab alongside Description, Details and Contact.

Also, looking for a specialised plugin for users who list a property to be able to add email address and fill out questionares from people who have visited their open homes and filled out their questionaire forms. It would email each user with a link to check the details they’d filled out. Any chance of something like this being possible, or being developed as an additional plugin?
