Edit add listing template

Hi guys. I’m using the business directory theme.

I’m trying to edit the add listing template to include a link to a page that contains a form ( see attachment). Upon submitting the form, users should be taken back to the add listing page. My questions:

1. What’s the template name that I need to edit? Is it achievable by adding custom css instead of changing the template?

2. How can I redirect users back to the add listing page after hitting the submit button?

3. Will date entered before clicking the link to the form be missed or be there after going back?
