Contact Form

What I would like to see in the contact form:

1. The Title like Contact Form above the name field.

2. A way to upload an image like a photo.

3 A way to customize the form and put an image in it.

and 4. A way to put more fields like a custom field there.

and maybe 5. A way to utilize it for testimonials and support pages too.

How can I do that?
