Database Structure Recommendation for Restaurant Directory

Hi,
We are planning the database structure for a restaurant directory.

These are the main fields:
FoodType (15 different types)
City (78 cities)
Setting (casual, elegant, and 5 more)

We also plan to add the following fields:
Telephone
Hours of Operation
Price Range (4 different ranges)
Physical location

Should we create 3 different CATEGORIES for the first three fields listed above (FoodType, City, Setting)?

Should we create a checkbox custom field named Amenities and include the rest of the items (Telephone, Hours of Operation, Price Range, Physical location) or simply create different fields for (Telephone, Hours of Operation, Price Range, Physical location)?

What about taxonomies?

What we plan is to have in the homepage three pulldown menus so that users can choose from the 3 different CATEGORIES for the first three fields listed above (FoodType, City, Setting).

First select FoodType, then they select City, and then they select Setting and click on Search button.

Any recommendations will be highly appreciated.
